What level should I sign up my child?  Learn about the FLHA program levels to determine what level you should sign up your player.

• Bantam, U14, Peewee, U12, Squirt, U10, Level 3 & U8 (7 & 8 years olds) - July 17, 2011
• Level 2 and U8 (6 years old and under ) - September 10, 2011
• Level 1 and Intro to Hockey - September 25, 2011
• Jr. Gold - November 20, 2011

BEFORE YOU BEGIN:  You must register first with USA Hockey (USAH) and then return to this Registration Session to signup for the 2011/2012 season.  USA Hockey requires that all players register on-line with USAH and obtain a USAH confirmation number.  This confirmation number is required during this FLH registration process in order to complete registration.

• Step 1: CLICK HERE to go to USA Hockey Registration and register your player(s). Be sure to PRINT and SAVE their confirmation code(s). Each player must have their own unique USA Hockey confirmation number.
• Step 2: Return to this registration and click on the Registration link below. During the session you will be asked to enter the USA Hockey Confirmation Number that you received in Step 1.

* Registration costs include league registration, home games, ref fees and membership fee.  Membership fee alone is $50.

**  All team costs are estimates and will vary based on the number of player per team and actual costs incurred.  A preliminary team budget will be distributed to each team/team manager in October and a final bill towards the end of the season.  Any additional cost or refund will then be collected/distributed via each teams manager.

• All registration will take place online via our website.
• All payments will be collected via Visa, Master Card, Discover, PayPal or debit card
• Late fee of $50 per player for returning players who register late.
• Registration fees do not include jersey deposit, volunteer deposit or team incidentals.
• Registration fees do not include clinic or tryout fee.
• Clinic and tryout fees are due upon registration.
• Clinic Fee is $125 and includes 15 hours of ice time beginning in late July and ending in late September.
• Tryout fees are still being determined and a separate registration will be required at a later time.
• Multi-player discount of $75 starting at the 3rd player and for each additional player. 
  • 3 players -$75
  • 4 players -$150
• Up to $125 can be offset by participating in association raffle. 

In order to participate in tryouts or your teams first ice session we must have the following:

• Birth Certificate
• Volunteer Check
• Consent to Treat Form attached below

Additionally, those registering from outside our Association boundries must have a completed Waiver form before participating in any Forest Lake Hockey Activities.
