• All payments will be collected via Visa, Master Card, Discover, or a checking account setup during registration. 
• No offline payment methods will be accepted.

SPECIAL RULES PERTAINING TO TRAVEL TEAMS

• A portion of the registration fee for Jr. Gold, Bantam, 15U, Peewee, 12U, Squirt, & 10U players are estimates and will vary based on the number of players per team and actual costs incurred.  A preliminary team budget will be distributed to each team manager in October and a final bill towards the end of the season.  Any additional cost or refund will then be collected/distributed via each team's manager.
• The registration fee does not cover incidental team expenses and is extra
• Registration fees listed below do not include the following:
  • Jersey fees for groups 10U/Squirt & older
  • Tryout fees for groups 10U/Squirt & older
  • 2023 Fall Clinic Fee
• Players registering from outside our association boundaries must have a completed waiver form before participating in any Forest Lake Hockey Activities.
• In order to participate in tryouts or your teams first ice session we must have the following:

  • Birth Certificate

  • DIBS Volunteer Deposit Check

• DIBS Requirments
  • Registration fees do not include the $600/Player or max $900/family DIBS volunteer deposit
  • All families must complete 8 hours/player or max 12 hours/family of dibs volunteer hours to not have your dibs check cashed.
  • Families with unfulfilled dibs hours as of January 31, 2024 will have their dibs check cashed & will be given refund for all completed hours at the end of the season

All past due invoices or fees must be paid in full to FLHA prior to registration.  Failure to pay them before registration will result in forfeiture of all fees paid during registration and your registration being cancelled.

• Bantam, 15U, Peewee, 12U, Squirt, & 10U - $250 - After July 31th
• Mite & 8U - $100 - After August 15th
• Jr Gold - No Late Fee

Bantam, 15U, Peewee, 12U, Squirt, & 10U

All player participating in Tryouts must have their registration, including tryouts, completed by September 15, 2023.  All players not registered for Tryouts will automatically be placed on the lowest level team for their age group.  

An additional tryout fee of $135 will be charged to any skater participating in tryouts.  Goalies participation in Tryouts is free.

Mite & 8U Level 4

All players wanting to participate in the Mite or 8U Level 4 evaluations must register for the Level 4 team during initial registration & have their registration completed by September 15, 2023.  All players not initially registered for level 4 will not be allowed to participate in the evaluations and will be placed in the appropriate level based on their current skill level.

There is no evaluation fee for the Mite Level 4 or 8U Level 4 team.

High School Tryouts

Any Hockey Association player that intends to tryout for a High School Hockey program while participating on a FOREST LAKE Hockey Association (FLHA) team or co-op/playing agreement team (i.e. the “Forest Lake/Chisago Lakes Rangers”), must complete and submit the Intent to Tryout Declaration form to be considered for a refund of the current years FLHA registration dues paid if they awarded a position on the High School team.

BEFORE YOU BEGIN:  

Using the link below, you must register first with USA Hockey (USAH) and then return to this Registration Session to signup for the 2023/2024 season.  USA Hockey requires that all players register on-line with USAH and obtain a USAH confirmation number.  This confirmation number is required during this FLH registration process in order to complete registration.

• Step 1: CLICK HERE to go to USA Hockey Registration and register your player(s). Be sure to PRINT and SAVE their confirmation code(s). Each player must have their own unique USA Hockey confirmation number.
• Step 2: Return to this registration and click on the Registration link below. During the session you will be asked to enter the USA Hockey Confirmation Number that you received in Step 1.

Players registering as goalies at the Squirt, 10U, Peewee, 12U, Bantam, & 15U levels will receive a $400 discount.

Bantam, 15U, Peewee, 12U, Squirt & 10U 

• All 1st year Squirt & 10U players are required to order jerseys.
• All new players to the association at the Bantam, 15U, Peewee, 12U, Squirt &10U are required to order jerseys.
• New Jerseys are optional for returning Squirt, 10U, Peewee, 12U, Bantam & 15U players
• Players must complete a separate jersey registration in order to purchase jerseys.  Registration will be sent out mid June.
• If not completed the player will not have jerseys to start the season and they will not have their name on their jerseys.

 Mite & 8U

• Jerseys for the Mite & 8U levels are incorporated in to the registration fee each player pays and will be provided in October or November.

Jr. Gold

• Jr. Gold players do not need to purchase any jerseys
• FLHA will provide a loaner jersey for each player for the season.
• A $160 Jersey deposit check will be required to receive your jersey and will be returned assuming the jersey is returned in good condition.

JR Gold players MUST register using the registration link below!
